ID Checking Verification Form

This pad is designed for the anyone checking ID's, specifically those checking at the door of a club.  If you receive a Misrep ID, the form is set up for the person presenting the ID to print their name and sign three times.  On the back of the form is for information gathering so you can make a case for the State Liquor Control.  Oregon Liquor Control Commission (OLCC) Inspectors have seen this form and love it.  Just another form to professional the bar security industry.
